Colorectal Cancer Diagnostic Assessment Program (CRC-DAP) offers patients easy and timely access to quality care. This centralized program creates a single point of entry into the cancer journey at diagnosis, improving access to cancer care, including expedited colonoscopy for patients whose physicians suspect colorectal cancer.

If you would like to refer your patient to the CRC DAP program, please complete the referral form and fax it to 416 603 5102. The Clinical Nurse Specialist will triage referrals and contact the patient within 48 hours of receiving the referral to conduct a telephone assessment, discuss next steps, and arrange for expedited diagnostic testing and consultation with the surgery team.

What We Do

The Colorectal Cancer Diagnostic Assessment Program (CRC DAP) was developed to improve patient outcomes and experience through enhanced access to services and coordination of care. The aim of the CRC DAP is to provide coordinated and expedited diagnostic services for all patients referred with a suspicion or confirmed diagnosis of colorectal cancer.

The DAP will facilitate the referral process via a single point of entry and triaging of patients by the Clinical Nurse Specialist, in collaboration with our surgeon team, to reduce wait times to diagnosis and cancer treatment, and provide quality patient care.

  • Centralized referral process to CRC DAP for specialized nurse navigator telephone assessment within 48 hours of referral, coordination of care, expeditious diagnostic testing and surgical consultation (within 2-4 weeks)
  • Clinical Nurse Specialist to oversee the diagnostic phase of treatment and organize appointments to confirm or rule out a colorectal cancer diagnosis
  • Providing patients and families with emotional support, guidance, information, and resources they may need to assist them during this stressful experience
